2. Reason and method of establishment

The joint-stock company Dopravní podnik města Hradce Králové with its registered office at Pouchovská 153, Hradec Králové 3 was established on the basis of the founding deed signed on 17 March 1997.

The reason for the establishment of the obliged entity is the performance of public services in the field of operation of urban public transport (bus and trolleybus transport). The sole shareholder of the obliged entity is the Statutory City of Hradec Králové. 11. Remedies

Appeal against a decision to refuse a request, complaint about the procedure for dealing with a request for information  

Appeals and complaints about the procedure for dealing with requests for information pursuant to Act No. 106/1999 Coll., as amended, are submitted within the deadlines laid down in this Act, in writing to the address of the registered office of the liable entity, or in electronic form to the address of the registry of the obliged entity. The Board of Directors of the Transport Company of Hradec Králové, a.s. decides on the appeal. The decision on the appeal cannot be appealed, the decision is reviewable by the court.

12. Forms

The form for a request for information pursuant to Act No. 106/1999 Coll., as amended, is not a condition for submitting a request for information.

13. Descriptions of procedures -  instructions for solving life situations

The request for the provision of information pursuant to Act No. 106/1999 Coll. is submitted on the day it was received by the Transport Company of Hradec Králové, a.s. The application must contain the mandatory requirements pursuant to Act No. 106/1999 Coll., in particular that the applicant seeks the provision of information within the meaning of this Act, and must relate to the competence of the Transport Company of Hradec Králové, a.s. The natural person shall state in the application the name, surname, date of birth, address of permanent residence or, if not registered for permanent residence, address of residence and address for service, if different from the address of permanent residence or residence. The legal person shall indicate the name, identification number of the person, registered office address and address for service if different from the address of the registered office. The obliged entity is not obliged to comply with a request for information in cases stipulated by Act No. 106/1999 Coll., especially in the following cases:

questions about opinions, future decisions and creation of new information (Section 2(4) of the Act)

protection of classified information (Section 7 of the Act)

protection of personality and personal data (Section 8a of the Act)

protection of business secrets (Section 9 of the Act)

protection of confidentiality of property relations (Section 10 of the Act)

further restrictions on the right to information (Section 11 of the Act)

If the application meets all the requirements and conditions for its admission, the obliged entity shall provide the applicant with information within 15 days of the date of receipt of the application or from the date of its supplementation. This period may be extended by 10 days for reasons laid down by law. If the obliged entity does not comply with the request, it will issue a decision rejecting the application within the deadline for processing the application.

Payment for the provision of information pursuant to Act No. 106/1999 Coll. is charged according to the tariff of payments, with the provision of information associated with this payment being subject to the payment of the required payment under the conditions laid down by this Act.
